Personal Care is provided by certified Personal Support Workers (PSW).

Services include:

  • Personal care, such as bathing and grooming
  • Assistance with special care as directed by health care professionals
  • Assistance with mobility and eating
  • Help with morning and evening routines
  • Limited homemaking services

These services are often put in place for people returning from hospital and on discharge the CCAC case manager or discharge nurse will meet with each patient to determine the level of help and support that they need to return and recover at home.

ONE CARE operates with a contract through Community Care Access Centre (CCAC) for services.CCAC assesses and makes referrals for individuals who need assistance with personal care.

Personal Support can be purchased privately. These services are the same services but can be purchased privately and can also be funded through Department of Veteran Affairs or Insurance.

Last year ONE CARE served over 1,300 clients in Huron and Perth and provided 127,000 hours of care. 

We know the value of the services in the feedback we receive from our clients.

Last year we had more than 70 clients nominate staff for our Client Care Awards. In the nominations the clients spoke about how they felt supported and comfortable. One client noted of a Personal Support Worker, “The worker is compassionately aware in her way of speaking, moving and carrying out my personal care. I have found her consistently in a positive frame of mind and always moving forward, focusing on bringing me back to a state of good health.”
