Assisted Living
ONE CARE’s Assisted Living program is designed to support high risk seniors to remain in living independently for as long as possible. This program is available in designated apartment buildings in Zurich, Brussels and Goderich. Admission to the Assisted Living program is directed by the SW LHIN.
Assisted Living supports clients who have significant need for assistance with the activities of daily living in order to remain living independently.

Care provided by certified Personal Support Workers (PSW)
Services include assistance with personal care, meal preparation, medication reminders, housekeeping and laundry. Frequent visits, both scheduled and unscheduled, are provided throughout the day.
Assistance is available 24/7 through an emergency call system, Lifeline.
Bookings – Monday to Friday from 8:30 a.m. to 4 p.m.
This program is offered in three small geographic hubs located in Brussels, Goderich and Zurich.

An intake assessment will be completed to determine the best type of service.
Available to older adults and adults with special needs in Huron/Perth County. This program is normally reserved for high risk seniors who require both scheduled and unscheduled visits throughout the day.
