At ONE CARE, we know how important it is to stay on top of your heart health—and we’re here to help. Through our free Blood Pressure Clinics, we provide a simple and supportive way to monitor your blood pressure and learn how small changes can lead to a healthier life.
How It Works:
During your visit, our trained volunteers will guide you through a quick and comfortable session, where you will:
Have your blood pressure checked using a highly reliable BP-Tru monitor
Complete a short health checklist to assess your risk for heart disease and stroke
Learn about simple ways to support your heart health by addressing key risk factors
At the end of your session, you’ll receive a copy of your results to take home. With your permission, we can also share them with your family doctor and pharmacist to help support your ongoing care.

Pharmacies
Churches
Community centers
CHAP sessions are often paired with our dining programs, so you can enjoy a nutritious meal while taking care of your health.
