Home At Last (HAL)

Assistance for a safe return home from hospital.

Home at Last (HAL) provides transitional support services for adults who are aging, frail, or have special complex needs and require assistance for a safe return home from hospital. 

Home at Last

Referrals are set up with the Ontario Health atHome hospital discharge planner or by contacting ONE CARE.

ONE CARE arranges services such as transportation, frozen meals and HAL attendant.

The attendant will meet the client at the hospital or at home.

  • The attendant can stay for up to four hours with the client.

  • Safety assessment of the home is completed.

  • Groceries or prescriptions can be picked up by the attendant.

  • The attendant can assist with, light meal prep housekeeping or laundry.

  • A complimentary meal (Frozen Meals on Wheels) is provided for each client.

Once the client is settled at home they will receive a follow up call within 24 to 48 hours.

Eligibility

  • Available to individuals 55 years and older (clients under 55 may be considered on a case-by-case basis).

  • Clients must reside in Huron or Perth County and be medically stable.

  • Requires a Home At Last Attendant to assist with the transition.

  • Available to residents of retirement homes and those transitioning to Available Retirement Care Home Enhanced Supports (ARCHES) Transitional Care beds.

  • Open to day surgery and elective surgery patients who require support upon returning home.

Service Availability & Registration

  • Available Monday to Friday, with weekend discharges possible if scheduled by Friday afternoon.

  • Scheduling hours: Monday to Friday, 8:30 AM – 4:00 PM.

  • Referrals require 24 hours’ notice, but same-day referrals may be accommodated when possible.

  • No fee for Home At Last services.
