Hospital to Home (LEGHO)

Bundled CSS Supports for Hospital Discharge and Community Stabilization.

The Let’s Go Home (LEGHO) program provides bundled Community Support Services (CSS) to assist older adults and people with health challenges as they transition from hospital to home. By offering free, short-term support, LEGHO helps individuals recover safely while reducing hospital readmissions and easing pressure on the healthcare system.

How LEGHO Supports Your Recovery

LEGHO provides access to essential community services for up to 6 weeks after hospital discharge, ensuring clients regain their strength and independence. A dedicated Care Planner conducts an assessment, provides weekly follow-ups, and connects clients with additional services to support recovery.

Core services include:

  • Care Planning & Wellness Checks – Regular follow-ups and intensive care planning, starting within 72 hours of discharge

  • Meals on Wheels – Nutritious meals delivered to support recovery

  • Transportation – Rides home from the hospital and to two follow-up medical appointments

  • Home Help – Light housekeeping meal prep and laundry

  • Caregiver Support – Resources and assistance for family caregivers

Eligibility Criteria

To qualify for LEGHO, individuals must:

  1. Be a senior or an adult with a disability

  2. Be at risk for:

    • Hospital readmission

    • Long-term care admission

    • An emergency department visit post-discharge

  3. Meet the following conditions:

    • Be medically stable for discharge

    • Be willing to accept services

    • Be discharged to a home setting (not long-term care, retirement home, or shelter)

    • Have a safe living environment

    • Reside in Huron or Perth County

    • Benefit from non-medical support during their recovery

Service Availability & Registration

  • Service Hours: Monday to Friday, with weekend discharges possible if scheduled by Friday afternoon

  • Service Area: Huron and Perth Counties

  • No fee for LEGHO services
