Our social work team provides compassionate support to help individuals navigate life's challenges and connect with essential resources.
Our services include:
✔ Mental Wellbeing & Coping Strategies – Guidance to improve emotional resilience and overall well-being.
✔ Community Connections – Assistance in accessing local supports and programs.
✔ Housing Support – Help finding appropriate housing, including subsidized and retirement homes.
✔ Short-Term Counselling – Supportive counselling to address immediate concerns.
✔ Government Applications – Assistance with funding applications and navigating available benefits.
✔ Power of Attorney Documentation – Guidance on legal documents and personal advocacy.
✔ Advocacy & Support – Ensuring clients receive the care and assistance they need.
Hours: Monday–Friday, 8 AM–4 PM
Service Area: Huron & Perth counties
Referrals: Provided by Ontario Health at Home
Cost: No fee
Our services are available to Ontario Health at Home clients who require social work support.
