Adult Day Program

ONE CARE’s Adult Day Program offers social, recreational, and therapeutic activities to support older adults in a group setting for part of the day. This program not only enhances the well-being of participants but also provides essential respite for family caregivers who provide day-to-day support.

Adult Day Program

Designed for older adults and individuals with special needs facing challenges accessing community programs due to physical frailty, disability, or chronic illness, the program fosters engagement, socialization, and independence while ensuring a safe and supportive environment. Assistance with daily living activities is also available as needed.

Program Activities & Features

Participants benefit from a structured daily schedule that promotes cognitive, physical, and emotional well-being. Activities include:

  • Morning discussion & coffee

  • Mental aerobics – Brain-stimulating exercises

  • Daily exercise program – Light physical activities

  • Full-course noon meal

  • Recreational activities – Bowling, bocce ball, bingo, dominoes, crafts, and musical entertainment

  • Large & small group activities – Adapted games to ensure all participants can participate successfully

  • Health monitoring – Monthly weight & blood pressure checks

  • Medication reconciliation – Helping clients and families maintain medication safety

Stroke-Specific Program

  • Tuesdays & Thursdays in Clinton are dedicated stroke-specific days

  • Clients from all of Huron County, South Bruce and North Brampton may be eligible to attend

Service Locations & Schedule

The Adult Day Program is offered in the following communities:

  • Clinton (Stroke-specific days: Tuesdays & Thursdays)

  • South Huron

  • Goderich

  • Grand Bend

  • Wingham

Service Hours: Monday to Friday, 8:30 AM – 4:00 PM (Schedule varies by location)

Eligibility & Registration

  • Open to seniors 55+ and individuals with special needs in Huron County

  • Participants must be able to attend and participate in a group setting independently

  • Referrals can be made through Ontario Health @Home, the Community Stroke Rehab Team (CSRT), or by calling 1-844-482-7800

  • An intake assessment will be conducted to determine the eleigability

Cost & Transportation

  • Adult Day Program fee: $13.00 per day

  • Transportation available:

    • $13.00 for in-town trips

    • $15.00 for out-of-town trips

  • Clients are invoiced monthly
