Assessments and Care Planning are provided by ONE CARE’s professional coordinators who can help determine which services best meet your needs. Resources available to caregivers can also be found in our Time to Talk section

Care Planners are responsible for assessing new and existing clients and developing and monitoring client-centered care plans.
Care Planning Duties include: RAI Assessments, Financial Assessments, working with complex needs and client situations, crisis management, participating in Health Links Coordinated Care Plans.
Care Planning is also offered for partner agencies.

Care Planning is offered to clients/caregivers who are struggling or have complex needs. Available to seniors over 65 & adults with disabilities in Huron/Perth County.
No available to those in long term care.
